Referral Criteria

Community health-care providers can refer patients to SickKids outpatient clinics using a secure web portal called EpicCare Link. Before you submit a referral, please review the clinic's referral criteria and guidelines.

This page currently lists referral criteria and guidelines for some of our outpatient clinics. Criteria and guidelines for the remaining clinics will continue to be added. Please continue to check our website periodically as updates will be ongoing.

Register for EpicCare Link

To register for EpicCare Link, you must first create an eCHN account. From your eCHN account, you will be seamlessly connected to SickKids’ EpicCare Link platform.

For urgent/same-day referrals only

Contact a clinic directly via SickKids Locating at 416-813-1500. On weekends and after 6 p.m. on weekdays, please contact the fellow on call via SickKids Locating.
